CYFN helps families develop team plans that require support from a variety of different individuals and organizations. CYFN already contracts with a broad range of community organizations throughout San Diego County to meet the needs of families identified in their family teams through the family team planning process.

If you are interested in registering your interest in becoming a CYFN contract agency to meet this need, please register your interest by completing the Registration of Interest Form. You will receive a confirmation of receipt if you are registering to meet future needs. If your organization and it’s array of services meets an unmet need identified by family teams, a representative of the Provider Relations Department will contact you to further discuss the provider application process.
